Preferably all 5 days, 07-3:30, but could be 10 hrs, flexible days
#Type of Cases: All cases General (Laparoscopic cholecystectomy/appy) , Robotic (Gastric Bypass, prostates), vascular (Endo AAA, A-V fistulas, CAEÕs), ortho, mastectomies, cystoscopyÕs)
# Initial name clear with CV in month/year format with explanations of any gaps over 30 days, any PLI info, 2 references if you have them, plus form (See Zenith). Followed by presentation template from Zenith once cleared.
